Rather than confirming the Impella’s position with imaging tools like echocardiography or fluoroscopy, as required by the deviceโ€™s instructions, the cardiologist moved the device blindly, without proper guidance or a guidewire.

The result? The device traveled into a major artery that feeds blood to the brain. Minutes later, the woman suffered a stroke. She lost the ability to move the left side of her body and could no longer speak.
